• At least one parent must be a baptised Catholic

• Parents must be committed to raising the child in the Catholic faith

• Parents’ Marriage Certificate (if applicable)

• Parents’ Baptism Certificates (if available)

• Godparents’ details

Each child should have at least one godparent.

Godparents must:

• Be at least 16 years old

• Be baptised and confirmed Catholics

• Be actively practising their faith
